Medical Alert Card

The Medical Alert Card was designed by the CdLS Foundation's Board of Directors whose members include parents of children with CdLS, medical doctors, and emergency medical service personnel. By just handing this completed document to EMTs, paramedics, or Emergency Room personnel, you save critical time. Please fill in this form as completely as possible, keep the information current, make a few copies, and be prepared to hand this to EMS personnel if they are ever needed.

For your child's safety:

• Print the Medical Alert Card (a free Adobe Acrobat download is available by clicking here)
• Make at least two copies of your completed Medical Alert Card
• Keep one copy in your home (preferably on your refrigerator)
• Give one copy to your child's school or care facility
• Keep one copy with your child (in a backpack or carrying case)
• Be sure to update changes in medication/conditions/doctor information
